Scholarship Criteria

• Completed application must be postmarked OR submitted online at www.dukecarrillofoundation.org no later than May 31, 2022
• Current senior enrolled in Flower Mound High School, Flower Mound, Texas
• Accepted for fall 2022 into an accredited college or university
• Weighted GPA of 3.5 or higher
• Provide copy of unofficial high school transcript to verify GPA requirement
• Varsity athlete
• Family history of military service
• History of community service including participation in Boy Scouts or Girl
  Scouts
• At least one letter of recommendation from individual of your choice

“Live Like Duke” Scholarship award will be distributed directly to the college or university in the recipient’s name.

If you are awarded a “Live Like Duke” Scholarship, please provide a photo and
short impact statement to be used by the Duke Carrillo Foundation in print
materials, on www.dukecarrillofoundation.org and/or the Foundation’s social
media and at Foundation events.

You will be notified no later than June 20, 2022 if you have been awarded a
scholarship.
